We are very excited to work with you! This instructional page contains very important information, so please read thoroughly. 

  • Trainer: Theresa Huyen, MD

    Time:

    • Tox & Filler @ 8:00 AM

    • MN & PRP @ 1:30 PM

    Location: Transform Aesthetic Center

                     821 Canal Street, New Smyrna Beach, FL 32168

    • For the Tox & Filler training, you will receive:

      • Xeomin/Botox 20 units

      • One (1) Filler Syringe

    • If you or your model would like to purchase more Tox & Filler:

      • Xeomin/Botox $9/unit

      • Filler $495/syringe

  • You are required to recruit your own model(s) for your training.

    Models must be 21yo or older.

    For Tox & Filler Session:

    • 1 Model for both

    OR

    • 1 Model for each (2 models total)

    For Micrneedling & PRP Session:

    • 1 Model for both

    • Models will stay in the waiting room until their procedure time to respect the semi-private environment, allow the training to flow smoothly, and respect limited space

    • Only training providers will be allowed in the treatment room

    • All participants will turn electronic devices to vibrate/silence to avoid distractions during the training

    • Log in to your online didactic course, review and complete all materials and pass the quiz prior to training

    • Recruit your own models for your hands-on session

    • Text the following info to our phone number (386-882-7830) so we can obtain consent for treatment from your model:

      • Full Name, DOB, Phone #, and Email

  • Yes! If you registered for either Tox & Filler or Microneedling & PRP and would like to register for both sessions, simply email Dr. Zengo at …

  • Although it is not required to complete and pass the online didactic, it is highly recommended as it will help you understand the methodology and techniques that you will practice during the hands-on.

  • Yes, it is important that you recruit your own model(s) for your training so you can follow up pre and post treatment. Seeing and knowing what the patient experiences also aids in your understanding of the treatments.

  • Once you provide us with your model’s info (name, DOB, & email), we will send them a Model Intake Form, Consent Form and Pre/Post Forms for them to review and sign.

    On the day of training, your model will arrive with a clean face (ie. no skincare, no makeup) and no jewelry as it is important to keep treatment and surrounding areas clean.

    We will notify the designated arrival time to your model prior to the training.
